About Jasper Dental Associates
On your first visit as a new patient (non-emergency in nature), you will receive a thorough examination of your teeth & gums with an oral cancer screening by the doctor. Current radiographs (x-rays) will be taken as they are an important part of this exam. X-rays allow the doctor to detect potential problems between and inside your teeth as well as the health of your gums and bone structure.
After the exam and consultation, we will discuss the results and recommend a plan for your preventive care (type of cleaning) with our hygienist and any required treatment. Adult cleanings are not normally done at the first consultation appointment, however, children are scheduled directly with the hygienist for their first visit.
We welcome your comments or suggestions. If you have any questions, please call. We are here to help you, your family & friends for years to come!
Treatment Estimates – Once a diagnosis is made by the doctor, financial estimates will be given to you before treatment is started. Our Treatment Coordinator will be happy to answer any questions you may have concerning fees, dental procedures and appointments.
Payment Guidelines – Payment for dental care received at Jasper Dental Associates is your complete financial responsibility. We request that payment is due in full when services are rendered unless other arrangements have been made in writing prior to receiving services.
Insurance Benefits – If you are fortunate enough to have a dental benefit plan to assist you in paying for treatment and will provide us with all the information necessary to file your claims, we will gladly complete the form for you and accept assignment of benefits from your carrier. This way, the amount due from you at your visit can be reduced by the amount you hope to receive from your dental benefit company for up to 60 days. We work with most major insurance carriers and participate in several Preferred Provider (PPO) programs, such as Ameritas, Assurant, Blue Cross Blue Shield, Delta Dental-Premier, Guardian, Humana, MetLife, Principal, United Concordia and United Healthcare. Call us to find out more.
Payment Methods – We accept cash, check (with a valid picture I.D. on file), MasterCard, VISA, Discover & AMEX. We also offer a patient financing payment plan through Care Credit, etc.

- I’ve been with JD for a few yrs. They are the best and can be trusted to be honest. I was told I needed some work - went to JD and they said no, if it starts to bother me they would do the work but until that time they will just watch it. Great to have someone who does Not want to do work on ya. - RM
